Find Your Italian Consulate

Last updated: September 15th, 2023

If you are in the United States where I am, the consulates are set up so that you must utilize the consulate in your jurisdiction. I was in Phoenix, so mine was Los Angeles. Notably one of the consulates with the longest wait times. At that time, the only way to make a reservation was over the phone to a 1-800 number that I think cost $70. But, lucky you, that is no longer a thing. Now you have the Prenota system to make your appointment online for free!  Especially if you live in one of the busier jurisdictions like New York, Chicago, Philadelphia, Los Angeles, you want to try to make your appointment as soon as possible.  With COVID, it's become much harder making the appointment.  I've seen some people say you need to try closer to the middle of the night to look for appointment openings.

The United States is split up into different areas which I've put together this map to show the jurisdictions. There are also links to each US consulate's citizenship pages along the left side of the WIKI so you get to it quicker.

Forms 1 through 4 that I went through

Recently I had someone ask via email what I mean by Form 1 through 4 when I mention it.  I wanted to respond to the email and did not have their email address, so I'll put the information here.

Each consulate will have their own forms which are basically the application forms. Wow!  It looks like the Los Angeles consulate has changed their website quite a bit and my links on the wiki no longer go to the citizenship page.  Apologies for that; I'll have to get those updated!

I'll show you on the Los Angeles consulate site; if you want me to guide you for your consulate, let me know which one it is and I can help - but the sites/process/forms are relatively the same.  Be sure not to use the Los Angeles ones (or the links here) if your consulate is not Los Angeles, they may not be accepted!!

For example, here is the citizenship by decent page:
https://conslosangeles.esteri.it/en/servizi-consolari-e-visti/servizi-per-il-cittadino-straniero/cittadinanza/citizenship-by-descent/

On that page, about a quarter of the way down, it starts talking about categories, or how many ascendants you have from your Italian born ancestor.

Each of these have a link that reads "CHECK LIST".  When you click into the checklist, I clicked the one on Category 1 and got this PDF:
https://conslosangeles.esteri.it/wp-content/uploads/2023/08/Check-List-CAT.-1-v.-07.08.2023.pdf

On page 3, on the first bullet point, they have links to "ITALIAN CITIZENSHIP REQUEST FORM CAT. 1 - 2" which was before referred to as "Form 1":
https://conslosangeles.esteri.it/wp-content/uploads/2023/08/ITALIAN-CITIZENSHIP-REQUEST-FORM-CAT.-1-2-v.-18.04.2023.pdf

And "Form 2" which is for you to list all of the places you've lived since you were 18 years old
https://conslosangeles.esteri.it/wp-content/uploads/2023/08/citizenship_by_descent_form_2_EDIT.pdf

Further down on bullet point 8, there's "Form 3" which is all of the living ascendants and where they had lived
https://conslosangeles.esteri.it/wp-content/uploads/2023/08/citizenship_by_descent_form_3_EDIT.pdf

And "Form 4" which is the same for anyone in your line who has already passed
https://conslosangeles.esteri.it/wp-content/uploads/2023/08/citizenship_by_descent_form_4_EDIT.pdf

I filled all of these out and had them notarized even though they don't have any place to.  I went to my bank and the notary just squeezed it into the bottom.
My only living ascendant, my father, had to get his notarized in Las Vegas where he lives.
